TECHNOLOGY, INC. the following forms must be submitted and mailed to the designated
financial manager.

Instructions:
1st - Fill out credit application form, then email the form
by clicking on the submit button within the appropriate form or mail in credit application
by printing it out.
2nd - Print out Term, credit and sales policy sign it and mail
it in. (Required for processing)
Credit application form - select from one of the following sources:
Term, credit and sales policy form - MUST be signed
and mail back to PTI for final approval.
